Enrolment Form
Prior to completing this enrollment form, please contact us directly at 3604485932 to confirm availability.
Enrollment
To enroll in lessons, students and/or parents must fill out, sign, and return a Student Enrollment Form prior to the start of the first lesson indicating that these Studio Policies have been reviewed and the student/parent agrees to abide by these guidelines.
Enrollment is for the studio calendar of 34 weeks per academic calendar year, September through June and summer lessons, July-August.
Enrollment is complete when the form is submitted and the yearly Materials fee of $35 per each student.
The Materials fee covers various sheets music, including recitals fees, refreshments, facility rental fees that you can continue to invite your family and friends to the performances.
Participation Expectations for Students
-
Arrive on time to your lessons.
-
Come with all materials-lesson books and all supplemental materials.
-
Ask questions and try your best.
-
Set aside a schedule practice time and make it as a part of your daily routine.
-
Five-Six practice session per week (of the same length) are recommended, so you can make notable progress.
-
Be prepared for each week’s lesson (practice/memorize the piece, completed theory)
-
Practice should be in a quiet well-lit room, uninterrupted, and on a reasonably maintained piano.
-
Students should practice for goals rather than minutes.
-
Lessons are designed to achieve goals each week.
-
Keep fingernails trimmed short.
-
Participate in performance workshops, master classes, piano performance exams, recitals (Piano Recitals -December/June).
I encourage parents to ensure that practice guidelines are being met.
You must have a piano or full-sized keyboard with 88 keys-weighted keys, foot pedal and a bench at proper height in your home. A piano is strongly recommended as nothing can take the place of its touch and sound. If you have an acoustic upright or grand piano, it should be tuned at least once a year. Maintaining your acoustic piano is important. I can recommend a piano tuner/technician.

2. You will need to purchase a metronome or download a free metronome app within the first month of lessons.
3. Flashcards (for beginner students).

Rescheduling Policies
If you need to cancel or reschedule your lesson, please notify me as soon as possible. To get a makeup lesson, you must notify me at least 24 hours in advance and only for illness or a family emergency. Any other missed lessons will be forfeited with payment required. Cancellation due to lack of preparation, school, forgetfulness, or social conflicts will result in a charge for the
lesson. As a busy professional it is usually very difficult for me to make up missed lessons.
Ample notice of rescheduling will provide more flexibility in options. If the student is late for the lesson, there will be no extra time to cover the entire lesson. Each student is guaranteed his or her regularly scheduled lesson each week unless the teacher is severely ill, there is an unexpected family emergency, or the teacher is on vacation. Lessons cancelled by the teacher will be
rescheduled.
Inclement Weather Policy/Illness
Piano lessons will remain on schedule. If students are not able to safely travel or attend the lesson, lessons will be offered using a video call otherwise the lesson is forfeited with payment required. Video lessons can be through FaceTime, WhatApp, Viber, or Zoom. Video Lessons can be effective alternatives when students aren’t able to be present in the studio. In the event of inclement weather, lesson times can often be adjusted on the scheduled lesson day to make travel easier.
Summer Study
Students are offered a summer term, no break. I request that all students take a minimum of six lessons during summer months (July-August.) I may schedule the lessons however is most convenient for both of us. Students that do not take the minimum lessons will be placed at the bottom of the list and may not be guaranteed a lesson time for fall, starting in September.

Termination of Services Policy
I will have to discontinue lessons if a student has an uncooperative attitude, persistently misses lessons, too many cancelations, fails to pay tuition on time. Other causes include but are not limited to, any violation of this studio policy. For any reason, if you must stop lessons, please give as much notice as possible.
